NHS Fast-Track Continuing Healthcare
For people with a rapidly deteriorating condition who may be entering the last weeks of life, the Fast-Track CHC pathway can fund care within 48 hours. Our team in Redbridge helps complete the Fast-Track Tool with your GP or hospital team and liaises with the local Integrated Care Board.

Yes. We provide both sleeping-night and waking-night cover. For people in their final days we can arrange two-carer cover where needed.
What happens after the person dies?
We stay with the family until the funeral director arrives, help notify the GP/community team, and connect families with Saint Francis Hospice bereavement support.
