NHS Fast-Track Continuing Healthcare

For people with a rapidly deteriorating condition who may be entering the last weeks of life, the Fast-Track CHC pathway can fund care within 48 hours. Our team in South Essex helps complete the Fast-Track Tool with your GP or hospital team and liaises with the local Integrated Care Board.

How quickly can you start care?

For fast-track palliative referrals we can typically start care within 24–48 hours of the first call. We’ll do the home assessment the same day where possible.

Do you replace the hospice or district nurses?

No — we work alongside them. Hospice and community nurses lead clinical care; we provide the constant presence that keeps your loved one comfortable and safe at home.

Can a single carer stay overnight?

Yes. We provide both sleeping-night and waking-night cover. For people in their final days we can arrange two-carer cover where needed.

What happens after the person dies?

We stay with the family until the funeral director arrives, help notify the GP/community team, and connect families with St Luke’s Hospice bereavement support.
